Checklist for Certification Reimbursement for Eligible Children and Spouses



STEP   COMPLETE?
1 Send a copy of your test results to the Department of Veteran Affairs (VA) office that handles your education benefits.  
2 Along with your test results, attach the following:.  
      A brief letter requesting reimbursement with the following information:  
          Your name and Social Security number
        or VA Claim number.
 
          The name of the test you took and the
        date you took it.
 
          Name and address of the organization
        issuing the license or certificate (not
        necessarily the organization that
        administered the test).
 
          Cost of the test (Registration fees,
        preparation guides, processing fees, etc.,
        will not be reimbursed).
 
          The following statement: "I authorize
        release of my test information to VA."
 
     
If you have never previously filed a claim for VA education benefits, you will also need to submit an application for benefits. Follow the steps below:
3 Go to www.gibill.va.gov and click on Education Forms. Scroll down to find VA form 22-1990. You may print a hardcopy by clicking Adobe PDF or complete the application online by clicking VONAPP. If you are unable to access this form, call 1-888-GIBILL-1 (442-4551) and request the form.  
4 Submit application either online or your nearest VA Regional Office.
